BURRILLVILLE – A 33-yeas-old Burrillville man died in an incident at his Smithfield workplace last week after he got caught in the hopper of a cement truck.
Matthew Maynard was severely injured while working at Greenville Ready Mix last Thursday, Oct. 21, and later died of his injuries, according to a release from the Smithfield Police Department.
Maynard, a father scheduled to be married to his fiancé during the upcoming Halloween weekend, was reportedly working on the vehicle. The Cedar Swamp Road business specialize in ready mix concrete, sand and gravel, colored concrete products and masonry supplies.
The Occupational Safety and Health Administration is investigating the incident.
Maynard’s family has set up a GoFundMe page to help support his son.
